REGISTERED NURSE/CARE COORDINATOR (STREET MEDICINE)
Essential duties and responsibilities: include the following. Other duties may be assigned.
- Performance key targeted physical assessment on patients as determined necessary and/or designated by a provider.
- Works with the Street Medicine team to improve outcomes of the Hepatitis C Elimination program.
- Performs resuscitative procedures.
- Screens patients for appropriate information. Instructs patients in the education of samples and tests.
- Maintains and reviews patients’ records, charts, and other pertinent information.
- Collects labs needed to initiate treatment, administers immunizations, and follows up on lab work in the street medicine setting.
- Provides assessments and patient education around treatment options, medication readiness, and ways to reduce the risk of reinfection and transmission.
- Responds to requests from providers regarding resource-finding.
- Develops resources for addressing emerging problems (ie, low-cost pharmacy lists, dental resources, patient education material, etc.)
- Works with providers to develop action plans based on goals generated from Practice Management System Reports
- Follows up with High-risk patients weekly or biweekly to support with any difficulty with adherence, including medication delivery and motivational interviewing.
- Responsible for obtaining prior authorization processes for patients.
- Upholds goals, policies, procedures, and expectations of CHASS Center, Inc.
- Maintains patient confidentiality by complying with HIPAA standards.
- Collaborates with members of the health care team and other health professionals

Computer skills: To perform this job successfully, an individual should have basic computer knowledge.
Education: Graduate from an accredited school of nursing and current licensure by the Michigan State Board of Nursing as a Registered Nurse.
Experience: Working with culturally, racially, and economically diverse individuals in a community-based organization. Work experience in helping unhouse individuals and/or Street Medicine environment preferred.
Other skills and abilities:
Ability to speak fluent Spanish preferred.
Physical demands:
While performing the duties of this job, the employee is regularly required to stand, walk, talk, and hear. Some bending, lifting, and stretching are required. Working under stress and the use of a telephone is required. The employee is frequently required to sit and use their hands to touch, handle, or feel. The employee is occasionally required to reach with hands and arms; climb or balance; stoop, kneel, crouch, or crawl; and taste or smell. The employee must occasionally lift and/or move up to 50 pounds. Requires working under stressful conditions and working irregular hours.

While performing the duties of this job, the employee is exposed to a normal primary care clinic environment, occasionally exposed to fumes or airborne particles, and toxic or caustic chemicals. The noise level in the work environment is usually moderate. Working under stress and using a telephone is required. Manual dexterity required. Frequent exposure to communicable diseases, toxic substances, medical preparations, and other conditions common to a clinic environment. Goes out in the community with the Street Medicine team.
