MEDICAL
REGISTERED NURSE/CARE COORDINATOR (STREET MEDICINE)
Work with the CHASS providers in delivering population-based primary health care to complex patients, who have inadequate resources for meeting their health needs, or who need extra assistance in achieving their health-related goals, specifically, patients who are on the Street Medicine Clinical outreach. Utilizes the nursing process of assessment, planning, implementation, and evaluation of the health system to collaborate, delegate, provide, and document care, specifically with the goals and objectives of the Hepatitis C Elimination Pilot program.

The Community Health Worker (CHW) is an integral member of an interdisciplinary health care team. The CHW will work closely with patients, the health care team, social services providers, and community partners to effectively manage the care of patients. Organizes and facilitates care activities and promotes self-management by advocating for, empowering, and educating the patient. Ensures safe, appropriate, non-duplicative, and effective integrated care. Must deliver health care-related services that are of high quality, efficient, and cost-effective, and professional manner. Provided services must be patient-centered and promote self-care and patient self-management.
