ADULT CARE
CHASS provides comprehensive preventive and primary care services to people 18 years and older. Our providers take the time to get to know you and work with you to develop your own personalized care plan. We want to be a true partner in your health.
Services include:
- Educating you on lifestyle choices that promote good health
- Focusing on preventive health care services including age-appropriate screenings, vaccinations and counseling
- Maximizing the treatment of chronic medical conditions such as high blood pressure, diabetes and high cholesterol with the goal of slowing, halting or reversing disease progression
- Lab services including hemoglobin, cholesterol profile, hemoglobin-A1c (diabetes check), lead and urinalysis are performed on-site at CHASS Center
- EKGs, bilirubin screening and pulmonary function studies in our Center, as well as some ultrasound services.
- Blood tests needing extra analysis are drawn in our lab and sent to the lab at Henry Ford Hospital.
We accept most insurance plans. For those without health care coverage, services are provided on a sliding fee scale based on household size and income. Payment for services rendered is expected at the time of visit, however, a minimum payment is required.
