Your electronic health record contains all of your medical information documented as part of your care at CHASS Center. You can conveniently obtain copies of your information by visiting our Medical Records Department.

What’s in my medical record?
- Results of lab tests, diagnostic studies and medical treatment you received while treated at CHASS Center.
- Health information collected by nurses, medical assistants and other members of your health care team such as notes from examinations, treatment and medications.
Can I obtain my medical information over the phone?
No, not from the Medical Records Department. The Medical Records Team can only provide your printed medical record. If you would like to discuss health information, such as diagnoses or lab results, we can send a message to your Health Care Team to follow up with you by phone.
Can I obtain my child’s record if they are over the age of 18?
No or unless you are your adult child’s legal guardian. In that case you must provide appropriate documentation that you are their guardian and follow the process outlined above.
Can my spouse/relative/friend pick up my medical record for me?
Yes, but only if the signed authorization form specifies that person may pick up the record for you. There is a place on the form to write the name of that person or any other that you designate to pick up for you.
Our patients can also securely obtain medical information and request appointments through MyChart, a convenient online patient portal.
