• Insured patients will need to bring insurance card to each visit to ensure coverage.
• Uninsured patients must bring in proof of income to qualify for the sliding fee discount. 
• Payment is expected at the time of service for all uninsured patients. 
• If you have insurance and are expected to pay a co-pay and/or deductible, that amount is due on the date of service.

We know that it is best to budget in advance for any planned visit to your provider. If you are unsure of the amount for an upcoming service, please contact our billing department at 313.849.3920 ext. 5022 to discuss. Our Customer Service Representatives will not be able to give you that information while checking in for your appointment.
